Claude Code 2.1.198 auto-continued past human prompts with no changelog note

Olaf Alders documents that Claude Code 2.1.198, shipped July 1, 2026 with no changelog or docs entry, made the AskUserQuestion tool auto-return 'proceed on best judgment' after 60 seconds when no human answered — and a half-filled dialog got submitted, not discarded. Anthropic reversed it in 2.1.200 two days later, but the episode is proof that auto-update can land undocumented default changes daily, a live risk for anyone running unattended agents on deploy-relevant prompts.
